The acknowledgement of this waiver via signature of the parent/guardian representing the player is required in order to participate in the 1st Annual Rising Stars 3 on 3 Classic Basketball Youth Tournament for Grades 5th - 8th.

By signing below, I, the parent/guardian acknowledge that I am aware of and accept the risks involved in my child’s participation in this activity and hereby release, discharge and hold harmless the TR White Sportsflex, its board members, tournament officials, tournament organizers, tournament directors, the Tyler Matthew Curtis Hill Foundation (TMCHF) committee and members, and other participants from all claims, demands, actions, judgments, and executions which my child, as well as the signed parent/guardian’s name on the registration form ever had, now has, or may have or claim to have.  This includes all personal injuries, known and unknown and injuries to property, real or personal, caused by, or arising out of the above mentioned basketball tournament.

I, the parent/guardian signing the waiver form have read the above contents and understand all its terms. I execute it voluntarily and with full knowledge of its significance.
