• Tennessee, Illinois or Ohio resident

• Active athlete in Football, Basketball or Track

• GPA 2.75 or higher   

• Male or Female

• Copy of High School Transcripts

• Copy of Acceptance Letter

• 1 Letter of Recommendation from any of the following: 

  • A high school coach 

  • A minister, community leader, teacher or guidance counselor

• Typed 250 Word Essay
